Overview

Qualified Social Worker – Adults Hospital Discharge. Location: West Yorkshire. Pay Rate: £35 per hour. Contract Type: Locum (Flexible Agency Role).

Are you an experienced and passionate Social Worker with a background in hospital discharge? Would you like to join a supportive and forward-thinking team where your expertise directly impacts patient recovery, independence, and wellbeing?

We’re working with a local authority in West Yorkshire seeking skilled Qualified Social Workers to join their Hospital Discharge and Home First Team. This is a fantastic opportunity to support people transitioning from hospital to home, while working in a fast-paced, multidisciplinary environment. Take the next step in your social work career while making a real difference to those recovering from hospital care.

About the Role

As part of the Hospital Discharge Team, you’ll help facilitate timely, safe discharges from acute settings — ensuring the right care and support is in place from day one.

You will:

  • Manage a caseload of adults with complex needs
  • Carry out strengths-based assessments and create tailored care and support plans
  • Deliver statutory responsibilities, including Care Act assessments, reviews, and safeguarding interventions
  • Work closely with health professionals, therapy teams, discharge coordinators, and care providers
  • Prioritise early intervention, recovery, and independence
  • Participate in a duty rota (1 week in every 4)

About You

To be successful in this role, you’ll need:

  • A recognised Social Work qualification (BSc, BA, MA, DipSW, CQSW)
  • Registration with Social Work England
  • Hospital discharge experience or equivalent acute-based adult services background
  • In-depth understanding of the Care Act, Mental Capacity Act, and safeguarding frameworks
  • Experience of managing a complex caseload and working under pressure
  • Strong communication, assessment, and decision-making skills
  • A commitment to person-centred, rights-based practice
