Overview

As a Primary Care Nurse your responsibilities will vary. You will work within a multi‑disciplinary team to deliver a nurse‑led service and prevent hospital admissions, undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients and undertake reception screening. You will use your skills and knowledge to deliver nurse‑led clinics to your patient group whilst being supported by a multi‑disciplinary team.

  • Work within a multi‑disciplinary team to deliver a nurse‑led service and prevent hospital admissions.
  • Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening.
  • Use your skills and knowledge to deliver nurse‑led clinics to your patient group while supported by a multi‑disciplinary team.
  • Provide outstanding evidence‑based care to patients, seeking out, listening to and acting on their feedback to personalise care.
  • Deliver non‑judgemental care that makes a real difference.

Working Hours
  • Monday – Sunday service.
  • 37.5 hours per week.
  • Day shift: 6:45 am – 7:45 pm, 1 weekend a month.
  • Night shift: 7:00 pm – 7:30 am, 3–4 nights a month.
  • 4‑week rolling rota.
Salary & Support
  • Full‑time position with an annual salary between £34,428 – £41,659 FTE, depending on experience.
