Job summary

York and Scarborough Teaching Hospitals NHS Foundation Trust is recruiting a Discharge Liaison Officer to join the Patient Flow team. You will work across acute medical, surgical or elderly wards to optimise discharges and support patients and staff with discharge plans.

The role requires strong communication, organisational skills and the ability to work under pressure in a collaborative, multidisciplinary environment. Weekend rota participation is expected.

Qualifications

• Educated to GCSE Level or equivalent. English at C or above – or equivalent qualification/demonstrable experience.
• NVQ Level 3 or equivalent experience.
• Knowledge and understanding of ward routines and discharging patients.
• Experience engaging with service users and stakeholders at different levels.
