Job summary

East Cheshire NHS Trust in Macclesfield Hospital is seeking a Patient Flow Discharge Coordinator to help ensure safe, timely discharges and a smooth patient journey.

You will work with multidisciplinary teams, monitor discharge planning, and take early action to reduce delays while maintaining high-quality, patient-centred care. Enhanced DBS is required for this role, and you will contribute to improving patient flow across the ward and hospital.

Qualifications

• GCSE English and Maths at minimum level
• NVQ Health Care Level 2 and 3 completed
• Care Certificate completed or in progress

Responsibilities

• Facilitate early discharges including use of discharge lounge where safe
• Act on MDT board rounds and discharge planning info
• Coordinate services to avoid delays in transfers of care
• Expedite referrals/assessments and raise concerns
• Collaborate with discharge doctor and complete electronic discharge notification
• Maintain communication with patient flow team about imminent/future discharges
• Ensure documentation supports safe discharge processes
• Review medical outliers with MDT in surgical wards
• Support bed/site management to update discharge profile daily
• Arrange transport and ensure vacated beds are used promptly
