Berkeley Place Surgery in Cheltenham is a friendly and supportivepractice providing a high level of quality clinical care to over 18,000patients.

We are looking to recruit an additional part time experienced Practice Nurse to join our team.

Main duties of the job

The purpose of the role is to provide assessment, screening, treatment services and provide health education advice to the patients.

Strive to improve standards of care and be involved inidentifying practice population needs.

The clinical team consists of five GP Partners, six Salaried GP, two ACPs, two Paramedics, three Practice Nurses, two HCAs and one Phlebotomist as well as theextended team of pharmacists, social prescribers, first contact practitioners.They are supported by a committed administrative and managerial team.

As a forward-thinking and innovative practice, we require similarlymotivated and enthusiastic Practice Nurse.

Job responsibilities
  • Assist GP's to help deliver effective implementation of management of pts withchronic diseases as detailed by the Quality and Outcomes framework.
  • Support the implementation of evidence-based guidelines in conjunction with thepractice nursing team.
  • Encourage the development of integrated services within Primary Care to includeintegrated practice based teams and multidisciplinary working.
  • Promote your own continuing professional development .
  • Recognise the opportunities for working in partnership with the Primary Healthcare team.
  • Participate in an annual appraisal.

CLINICAL

Support the practice in achieving quality indicators and targets of the GMScontract and any relevant National Service Frameworks.

Experience in Chronic Disease Management or willingness to undertake training.

Undertake opportunistic screening when clinically required. Perform venepuncture andelectrocardiography

Undertake wound care/ management/dressing techniques.

Legulcer management, dopplers and compression bandaging.

Carry out ear examination and syringing, following appropriate training andassessment.

Perform cervical screening and promote breast awareness.

Administer adult and paediatric injections under an individualised prescriptionor Patient Group Direction, ensuring safe storage, rotation and disposal ofvaccines and drugs.

Provide advice on lifestyle issues, assist and support patients in changingtheir unhealthy behaviours and refer appropriately.

Provide an adult and childhood immunisation service working to Patient GroupDirections - following appropriate training if required.

Deliver travel health care, including vaccinations and advice for patients -following appropriate training if required.

Recognise the signs of child abuse, family violence, vulnerable adults,substance abuse, addictive behaviours, suicidal tendencies and be aware oflocal, referral procedures.

Apply infection control measures when collecting and handling of laboratoryspecimens, disposing of waste materials, dealing with blood and body fluidsspillages and report and treat sharps injuries.

Undertake first aid and management of emergencies, eg burns, haemorrhage,hypo/hyperglycaemic emergencies/asthma attacks.

Undertake annual training and demonstrates competency in anaphylaxis andresuscitation techniques Research, Audit and Quality

Contribute to the provision of equitable, high quality services for patients inthe locality.

Helpsupport the integration across primary, secondary and tertiary care services.

Maintain an interest in national research initiatives and disseminateappropriate information to health professionals if asked to do so.

Ensure all care is evidence based using up to date validated literature.

Contribute to research and development programmes where required to improveclinical practice.

EDUCATION AND TRAINING

Following identified training needs, participate in training and facilitationof other practice nurses & health care assistants.

Provide mentorship and guidance for health care assistants

Participate in partnership programmes of awareness raising, detection andprevention of chronic diseases.

Maintain a level of expertise in specific chronic disease management (whenappropriate) and other pertinent areas of health care, and seek to develop newand improved skills.

Advise members of the public on the concepts of health promotion, education,prevention and protection

Seekout new knowledge of nursing and health by reading, enquiring and partaking incontinuous education programme.

Takeresponsibility for personal development and education including in servicetraining.

Direct, supervise and help mentor nurses and health care assistants in theclinical setting.

Actively participate in multidisciplinary education and training.

Monitor and maintain consistency of professional standards and take appropriateaction if standards are not met.

Maintain professional development by attending relevant courses and study daysto keep abreast of current knowledge and developments.

Abide by the legal requirements and statutory rules relating to practice andrecord and maintain periodic professional registration with RCN & NMC

Participate in an annual appraisal.

Completing all mandatory training modules as specified by the Organisation

REGISTERED HEALTH PROFESSIONAL

Maintain registration with NMC/ Medical Indemnity providedby the Organisation.

Where the appropriate professional organisation details arequirement in relation to supervision, it is the responsibility of the postholder to ensure compliance with this requirement. If you are in any doubtabout the existence of such a requirement, in the first instance advice shouldbe sought from the Lead Clinician.

RISK MANAGEMENT

It is a standardelement of the role and responsibility of all staff that they fulfil aproactive role towards the management of risk in all of their actions. Thisentails the risk assessment of all situations, the taking of appropriateactions and reporting of all incidents, near misses and hazards.

RECORDS MANAGEMENT
