Qualifications

• Band 5 or Band 6 nurse with experience in clinical environments.
• Demonstrated excellent clinical and communication skills.
• Ability to adapt to different software systems.

Responsibilities

• Provide enhanced palliative care and support to babies and children.
• Coordinate care between hospice, hospitals, and community teams.
• Attend MDTs and share updates on patients.
• Facilitate training across clinical environments.
