Job summary

Changing Lives in Doncaster invites applications for an Outreach Support Worker. You will join a dedicated team delivering accommodation and support to individuals with complex needs, with training provided to equip you for the role.

Shifts run across early morning to evening (6am–8pm) to ensure uninterrupted support. A compassionate, person-centred approach and teamwork are essential for success in this role.

Qualifications

• Experience in outreach or housing support is preferred.
• Ability to work flexible shifts spanning early morning to evening (e.g., 6am–8pm).
• Commitment to compassionate, person-centred support.
• Training will be provided on all aspects of the role.

Responsibilities

• Provide accommodation support to individuals with complex needs.
• Deliver high-quality, reflective support and participate in case management reviews.
• Collaborate with a multidisciplinary team to ensure continuity of care.

About the Service - Housing & Homelessness Services

A safe home is the solid base from which you build your life. We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people.

Accommodation to us doesn’t just mean a roof over your head and shelter from the elements. It needs to be a welcoming and flourishing environment, offering opportunities for all those we support to feel comfortable and develop. About the Job

Welcome to the Outreach team located in Doncaster, where we have been operating for several years. Our project is dedicated to providing accommodation for individuals with a range of complex needs. As a Support Worker you will have the opportunity to join our passionate team of staff here in Doncaster

We are committed to delivering high-quality support, and we welcome your expertise and dedication to help us continue making a positive impact in the lives of those we serve. You will be given training on all aspects of the role as well as taking part in regular reflective practice and case management reviews with the whole team.

Our project operates between the times of 6am - 20.00pm so commitment and flexibility are essential as shift times may vary to ensure uninterrupted support.
