Job summary

Changing Lives in Newcastle is seeking a dedicated Support Worker to help individuals access housing-related support services, navigate complex needs, and work towards independent living. The role includes training on all aspects of the job, reflective practice, and regular case management reviews with the team.

You will join a small team in Newcastle Clusters, operating from Walker, where 44 properties are managed for people facing homelessness, mental health, and substance challenges.

Responsibilities

  • Build productive relationships with individuals accessing the Changing Lives service.
  • Support, listen, guide and signpost people who use our service, to allow them to receive the required support to manage their specific needs.
  • Ability to build relationships on 121 bases, to provide key support to people to enhance their personal wellbeing.
  • Support people to attend appointments with other professionals and within the community.
  • Carry out regular building health and safety inspections, identifying risk factors, responding accordingly, and carrying out basic repairs where required.
  • To ensure that the standard of the properties are maintained by following designated tasks, whilst maintaining a high level of cleanliness, order and sanitation meeting all legal and statutory requirements to operate safely, effectively and efficiently.
  • Empower people to improve their life skills through signposting to external agencies who can support in this area.
  • Ensure that full written reports, documentation and systems are updated daily, accordingly in line Changing Lives policies and procedures, whilst always maintaining a high level of confidentiality and data protection.

About the Service - Housing & Homelessness Services

A safe home is the solid base from which you build your life. We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people.

About the Job

Newcastle Clusters is located in Walker, where we have been operating for 15 years. Our project provides 44 fully furnished properties for individuals who are facing a range of complex needs, ranging from homelessness, mental health and substance addiction.

We support individuals and families with small children and babies.

As a Support Worker, you will have the opportunity to join our passionate team of 4 members, which includes a dedicated group of 3 support workers.

We are committed to delivering high-quality support, and we welcome your expertise and dedication to help us continue making a positive impact in the lives of those we serve. You will be given training on all aspects of the role as well as taking part in regular reflective practice and case management reviews with the whole team.

The Support Worker plays a crucial role within Changing Lives, providing information, advice and guidance. Undertaken via referral and direct individual support, the Support Workers aim is to enable the person to live independently and work towards overcoming the interconnected problems that lead them into accessing our services.

Your key activities will include, but not exclusive to:
