Job summary

Changing Lives is seeking a dedicated Support Worker for our 5-bed Housing & Homelessness service in Bishop Auckland. You will provide person-centred support to women, helping them build skills, confidence and independence in a safe, respectful environment.

You will contribute to individual support plans, manage safeguarding, assist with housing applications, budgeting, benefits, health and wellbeing, education and employment access, and collaborate with local authorities and health partners to

Responsibilities

• Create, implement and regularly review individual support plans and risk assessments.
• Support clients with housing applications, tenancy sustainment, budgeting and benefits.
• Promote health, wellbeing, education, training, employment and access to community services.
• Collaborate with local authorities, housing providers, healthcare professionals and other support services to achieve positive outcomes.
• Maintain accurate records and ensure safeguarding responsibilities are met.
• Encourage service users to maximise their independence.

About the Service - Housing & Homelessness Services

A safe home is the solid base from which you build your life. We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people.

About the Job

We are looking for someone to join our 5 bed, Housing and Homeless women's service, Teesdale House, who will provide person-centred support to the women in our accommodation, empowering them to develop the skills, confidence, and resilience required to achieve and sustain independent living. Responsibilities include creating, implementing, and regularly reviewing individual support plans and risk assessments, supporting clients with housing applications, tenancy sustainment, budgeting, benefits, health and wellbeing, and access to education, training, employment, and community services. You will work collaboratively with local authorities, housing providers, healthcare professionals, and other support services to promote positive outcomes, maintain accurate records, ensure safeguarding responsibilities are met, and encourage service users to maximise their independence.

The role is currently based in Bishop Auckland, with a potential relocation to the DH7 area in the immediate future.
