Geriatric Medicine Outpatients Nurse – NHS Hospitals

Location: Mid West Wales (NHS Outpatient Clinics)
Contract: Part time Flexible Weekend Work

SJK Healthcare is urgently recruiting an experienced Geriatric Medicine Outpatients Nurse to support specialist elderly care clinics within NHS hospitals. This role is ideal for a nurse passionate about delivering high-quality care to older adults with complex needs.

Key Responsibilities
  • Supporting consultant-led geriatric & frailty clinics
  • Completing comprehensive patient assessments, including cognitive and mobility reviews
  • Managing patients with common geriatric conditions such as:
  • Frailty & falls risk
  • Dementia & cognitive impairment
  • Multiple long-term conditions (multimorbidity)
  • Polypharmacy concerns
  • Mobility issues & deconditioning
  • Continence issues
  • Supporting coordination of tests, referrals & community follow-up
  • Providing patient and family/carer education
  • Supporting telephone/virtual follow-up clinics
  • Accurate record-keeping within NHS electronic systems
  • Working closely with Consultants, Frailty Teams, Physiotherapists, OTs, HCAs & Social Care teams
Requirements
  • Registered Nurse (NMC PIN required)
  • Minimum 1 year experience in Geriatrics, Frailty, General Medicine, or Community Nursing
  • Strong understanding of elderly care pathways
  • Excellent communication, compassion & patience
  • Ability to manage complex cases and vulnerable patients
  • Confident working both independently and as part of an MDT
  • You must live in the UK and have the right to work in the UK
