Job description

This position is open to staff working at the Trust in the 'ECT Family' this includes current East Cheshire NHS Trust employees, both NHSP and ECT bank staff, ISS staff and volunteers at the Trust. Unfortunately, we cannot accept applications from applicants not listed within these groups.

An excellent and exciting opportunity has arisen within our Acute and Integrated Care Community Services at East Cheshire NHS Trust. We are looking to recruit an enthusiastic and compassionate Registered Nursing Associate to join our Hospital at Home Team, to support the Virtual Ward and Urgent Community Response Teams, in collaboration with partners across health and social care within the East Cheshire region.

The role of the Nursing Associate is to work with the multi-professional team and the Registered Nurse contributing to the ongoing assessment, providing, and monitoring care and interventions in line with the service pathways and with supervision from clinician(s). They will help to monitor and support the health and social care needs of our people to prevent unnecessary attendance and admission to hospital, thus providing holistic and person-centred care and support in a variety of settings. Hours per week: 33.00, We welcome applications from Graduate visa holders; it is important to note that you must have a minimum of 6 months before this expires at the start of your employment for training and operational reasons. The Nurse Associate will work independently to implement interventions under the supervision of a Registered Nurse. To support the delivery of patient centred approaches and provide compassionate, safe and effective care to a designated caseload / cohort of patients. The Nurse Associate will provide holistic and person-centred care and support for people across the sphere of nursing and always communicating effectively within the team, with other health care professionals and with the patient family, carers / visitors. The Nursing Associate will work independently but with the support and leadership of Registered Nurses, working within aspects of the nursing process to ensure high standards and a safe environment are maintained. Travel is an essential part of this role, please demonstrate on your application form how you would meet this requirement.
