Weare delighted to advertise this rare opportunity for a highly motivated individual who has a passion for community care of the Neonate, to lead our team of Neonatal outreach nurses across the Leeds centre for Newborn care.

The transition from an intensive care setting to home is a process that takes careful planning and preparation. As the lead for Neonatal outreach, you will manage the outreach team who will guide and support the process of discharge in the ward setting to the home setting.

This role requires autonomous working therefore extensive Neonatal clinical knowledge is needed to aid assessments and decisions that you are making as well as supporting the team with their caseloads. You will be responsible for the management and overseeing of the outreach caseload, delegating, organising and supporting others workload as well as taking a caseload within this role.

Main duties of the job

The Neonatal outreach senior sister will provide support lead and manage a team of registered nurses and nursery nurses, ensuring the coverage of a 7-day service.

The postholder will work collaboratively with the multidisciplinary team to deliver care to Neonates transitioning from hospital to home, with the aim of earlier discharge if appropriate.

They will assess, plan and implement and evaluate care for Neonates requiring ongoing support in the community, acting as liaison and advocate between the families and other health professionals and services.

The postholder will serve as a specialist resource for both the Neonatal units and secondary healthcare providers, offering expert guidance and educational needs.

As the team leader you will manage and contribute to service development, training, audit and support quality improvement projects to improve evidence-based care to our patient population.

Independent decision making is integral to this role as a practitioner and team lead, to effectively coordinate the multiple aspects of care therefore excellent communication skills, autonomy and flexibility is essential.

The service is currently provided across two sites, Leeds General Infirmary and St James's University Hospital. We have a NICU and surgical unit at LGI, SCBU at SJUH, Transitional Care Unit and an Outreach Service.   • Initiate early discharge if appropriate to the patient
  • Collaborate & build relationships with Multidisciplinary teams
  • Act as a source of specialty expertise to parents & carers, Neonatal outreach and the Neonatal teams.
  • Provide all education teaching/training to parents & carers to facilitate discharges.
  • Participate in planning and implementation of quality initiatives and targets, in conjunction with the MDT team.
  • Drive and hold a current driving licence

Managerialresponsibilities

  • Lead and manage the outreach service, analysing and scoping to continually develop the service.
  • Promote good communication and working relationship with the other leads of the service.
  • Lead and manage projects in conjunction with the service and Matrons.
  • Have oversight of the team development, reviewing and supporting them with performance and attendance.
  • Act on compliments, complaints and feedback accordingly.
  • Involvement in clinical governance and risk across outreach and the Neonatal service.
  • Provide education for families to prepare them for home.
  • Promote and implement evidence-based practice.
  • Ensure an environment conducive to the educational needs of all staff and students.
  • Lead on programmes of standard setting and audit.
