A prominent health service organization in the UK is seeking a Neurorehabilitation Case Manager to enhance services for patients with complex needs. The role involves working with various stakeholders, reducing delays in patient journeys, and supporting fellow Case Managers. We are seeking to recruit a Neurorehabilitation Case Manager to work with a range of stakeholders to improve Specialized Rehabilitation services for patients with highly complex needs across the Midlands.
The postholder will need to have an excellent understanding of the specialised and core services that currently support this population and have a strategic vision for future requirements. They will be expected to work in partnership with a wide range of patients, carers and stakeholders and will be responsible for reducing delays in the patient journey associated with admission to and discharge from Specialized Rehabilitation Services.

Earlier this year, the Government announced that NHS England will gradually merge with the Department of Health and Social Care, leading to full integration. The aim is to create a smaller, more strategic centre that reduces duplication and eliminates waste.

Successful candidates will have a proven track record of performing using high levels of autonomy. We are looking for self-starters with specialist knowledge of the therapeutic and rehabilitation requirements for patients with neurological conditions, specifically sudden onset conditions. We are also looking for candidates with strong leadership skills and the ability to work closely with multi-agency partners to support some of the most complex cases in the service. These individuals will have a specific responsibility for driving transformation within Specialist Rehabilitation Services within the Midlands; they will also provide support to fellow Case Managers at local, regional, and national level.
