A public healthcare organization is seeking an Integrated Care Coordinator Service Lead in Norwich for an 18-month secondment opportunity. The role involves supporting and developing service delivery, guiding Integrated Care Coordinators (ICCs) to provide high-quality care, and ensuring effective team operations. Candidates should have experience in health or social care, possess strong leadership and communication skills, and a desire to improve community health services. Competitive salary and various benefits included.
We are looking for a motivated Service Lead for Integrated Care Coordinators (ICC) to support the delivery and development of local services, strengthen partnerships across health, social care and community services, and guide ICCs to provide high-quality, person‑centred care.
Integrated Care Coordinators (ICCs) play a key role in the NHS Neighbourhood Health model by joining up care and improving communication across GP practice, NHS organisations, mental health, social care and community services.
Working across Central Norfolk (North Norfolk, South Norfolk, and Norwich), ICCs follow shared standards while adapting to the needs of the communities they serve. ICCs identify people who need extra support, coordinate care from multiple professionals, and ensure individuals and their carers receive timely, seamless, and person‑focused care. By helping teams communicate and work together, ICCs support people to stay independent, reduce unnecessary hospital admissions, and help build healthier, stronger communities.
You will be a supportive leader who can motivate and guide a team, build good relationships across services, and help solve problems as they arise. You will bring experience working in health, social care, or a related field, bring excellent communication, leadership and problem‑solving skills. Have experience managing staff, navigating competing priorities and delivering service improvements. Confidence in using digital systems and a willingness to develop your skills further are important.
If you are motivated by improving outcomes for local people and building integrated ways of working across the system, we welcome your application.
