Job summary

Transform Housing & Support is seeking a Housing & Support Officer to work across West Surrey, in a community-based role with regular travel between sites. The role focuses on sustaining tenancies and enabling independent living through a Housing-Led approach.

You will join a supportive team, empower clients to resolve challenges, access health and community services, and promote harm reduction while maintaining professional boundaries.

Qualifications

• Ability to build professional, non-judgmental relationships with people with complex needs.
• Confident working with individuals experiencing drug and alcohol dependency.
• Effective communication with clients, colleagues and partner agencies.
• Ability to manage risk and escalate concerns when needed.
• Organised with ability to manage own workload.
• Full driving licence and access to a vehicle for work.
• Basic numeracy and written communication skills (GCSE Maths and English level 4).

Responsibilities

• Support clients to sustain tenancies and prevent homelessness.
• Deliver person-centred support aligned to individual plans.
• Assist clients to access health, substance misuse, employment and community services.
• Promote harm reduction and positive behaviour change.
• Carry out housing management duties including rent monitoring and property checks.
• Complete risk assessments and contribute to safeguarding processes.
