We are seeking a compassionate and organised individual to join our Hospice Rapid Access Service (HRAS) as a Care Coordinator. This role is vital in ensuring that our patients receive the highest quality of care by coordinating and managing their healthcare plans.

Salary: £27,000 - £30,100 depending on experience
Hours of work: Full-Time 37.5 hours per week
Contract: Permanent
The primary duties of the Care Coordinator include:
Developing and managing comprehensive care plans for patients.
Coordinating with healthcare professionals to ensure continuous and consistent patient care.
Monitoring patient progress and adjusting care plans as necessary.
Providing support and guidance to patients and their families regarding healthcare needs and services.
Maintaining accurate and up-to-date patient records.
Organising and scheduling appointments, treatments, and follow-ups.
Facilitating communication between patients, families, and healthcare teams.
Ensuring compliance with all healthcare regulations and standards.
We are looking for candidates who demonstrate the following:
Previous experience in a healthcare or care coordination role.
Strong organisational and multitasking abilities.
Excellent communication and interpersonal skills.
Proficiency in Microsoft Office applications (Word, Excel, Outlook).
Ability to work both independently and as part of a team.
Knowledge of healthcare regulations and best practices (desirable).
