Job summary

St Luke's Hospice in Basildon is seeking a compassionate Care Coordinator to join our Hospice Rapid Access Service (HRAS). You will coordinate and manage comprehensive care plans to ensure patients receive high-quality care.

The role requires strong organisational and communication skills, experience in healthcare, and the ability to work independently within a multidisciplinary team. Permanent, full-time position with salary £27,000 - £30,100.

Qualifications

• Previous experience in a healthcare or care coordination role.
• Strong organisational and multitasking abilities.
• Excellent communication and interpersonal skills.
• Proficiency in Microsoft Office applications (Word, Excel, Outlook).
• Ability to work both independently and as part of a team.
• Knowledge of healthcare regulations and best practices (desirable).

Responsibilities

• Developing and managing comprehensive care plans for patients.
• Coordinating with healthcare professionals to ensure continuous patient care.
• Monitoring patient progress and adjusting care plans as necessary.
• Providing support and guidance to patients and their families.
• Maintaining accurate and up-to-date patient records.
