About the Role

As a Care Coordinator, you will be responsible for coordinating care services and ensuring that care visits are delivered efficiently, safely and in line with client requirements.

Your duties will include:

• Planning and maintaining care staff rotas
• Coordinating and scheduling client visits
• Ensuring continuity of care wherever possible
• Liaising with clients, families, care staff and healthcare professionals
• Monitoring service delivery and responding to changes in client needs
• Supporting recruitment, onboarding and training activities
• Maintaining accurate records and electronic care systems
• Ensuring compliance with company policies and regulatory requirements
• Supporting the on-call service as required

What We Are Looking For

• Previous experience in a Care Coordinator role or similar position within Health & Social Care.
• Experience using electronic rostering or care planning systems.
• Excellent organisational and time management skills.
• Strong communication and interpersonal abilities.
• Ability to work effectively in a fast-paced environment.
• Good knowledge of domiciliary care services and client-centred care.
• NVQ Level 3 in Health & Social Care (desirable).
• Right to Work in the UK. We are currently unable to offer sponsorship.
