Job summary

Excelcare Holdings is seeking a Homecare Care Coordinator (Maternity Cover until Oct 2026) to manage referrals, rostering, and care plans for Tower Hamlets. The role involves liaising with local authorities, carers, and families to ensure service quality.

With 8:30am–5:00pm hours, on-call rotation, and benefits, this position offers a structured role in a busy East London home care service.

Qualifications

• Experience in Home Care setting preferred, especially as a Care Coordinator.
• Understanding of regulatory requirements in home care (CQC) is desirable.
• Excellent verbal and written communication and data accuracy.

Responsibilities

• Proactively manage new care package referrals.
• Allocate rosters and workloads for Homecare workers.
• Match workers to service users based on need, skills and personality.
• Maintain links with local authority, carers, service users and relatives.
• Record and report complaints and monitor quality standards.
• Oversee care packages and care plans per service agreements.
• Provide on-call coverage as required and participate in reviews.
