Job summary

British Red Cross is seeking a compassionate Support Worker to assist individuals discharged from hospital in Redhill and surrounding areas.

You will manage a caseload, provide emotional and practical support, complete welfare checks, shopping, and home visits, and respond to referrals from clinicians.

This fixed-term contract until March 2027 offers flexible working arrangements and a pro-rata salary of £24,479 per annum, with a driving licence required for travel around the area.

Qualifications

• Kind, calm, confident approach with great people skills.
• Ability to work independently and as part of a team.
• Full UK driving licence and access to a vehicle.
• Basic IT skills and a commitment to following procedures.

Responsibilities

• Manage a caseload to provide emotional and practical support for service users discharged from hospital, including welfare calls, shopping and home visits.
• Support adults after hospital stays to help them recover safely at home.
• Respond to referrals from clinicians and community health professionals.
• Assess needs and follow a personalised support plan for each service user.
