Job summary

British Red Cross is recruiting a Support Worker for Discharge to Assess in South Ayrshire with travel across the area. Salary £24,479 per annum based on 35 hours per week, or £13.45/hour. The role involves home visits, practical and emotional support, and preventing hospital readmissions to help people regain independence.

You will need experience supporting vulnerable adults, strong communication, and a flexible approach to hours including evenings and weekends. Closing date 4 October 2026.

Qualifications

• Experience supporting people with physical, sensory, or mental health needs.
• Strong communication skills and ability to work independently and in a team.
• Knowledge of NHS and social care services and how to access them.

Responsibilities

• Visiting service users at home to assess needs and tailor support plans.
• Assist with daily living activities such as bathing, dressing and meals.
• Liaise with health, housing and social care professionals for consistent support.
• Carry out welfare checks and help individuals regain independence.
