Responsibilities

• Managing a caseload to provide emotional and practical support for service users discharged from hospital, including welfare calls, shopping and home visits.
• Supporting adults after a hospital stay to help them recover safely at home.
• Responding to referrals from clinicians and community health professionals.
• Assessing needs and following a personalised support plan for each service user.
