Job description

Location: Sandwell (covering West Bromwich, Wednesbury, Tipton, Rowley Regis)

Hours: 37 hours per week, rolling rota

Salary: £24,242.20 per annum

Contract: 12 Month Fixed Term Contract

Additional Requirement: Full UK manual driving licence held for a minimum of 2 years and a willingness to drive service vehicles

We are looking for a dedicated and enthusiastic Support Worker to join our 7-day hospital discharge service in Sandwell. If you have a caring, patient, and helpful nature you could make a tangible difference to someone’s life. You’ll work closely with hospital teams and partner organisations to help people return home safely after a hospital stay and maintain their independence.

Our support workers are a friendly face to someone who may be struggling to do things for themselves due to age, illness, or vulnerability. What You’ll Do
  • Respond to referrals and complete welfare calls and home visits.
  • Attend hospital wards and discharge units to support patients.
  • Provide practical and emotional support to aid recovery and independence.
  • Assess service users’ needs, complete and follow support plans, identify safeguarding concerns, and accurately record case notes.
  • Signpost individuals to appropriate services and community resources.

This is a hands‑on, community‑based role where no two days are the same. You’ll need flexibility and confidence working in different locations across Sandwell.

To be a successful Support Worker, you’ll:
  • Have a full UK manual driving licence.
  • Have a good knowledge of services provided by the NHS and Social Care.
  • Contribute to the ongoing improvement of service quality and user outcomes.
  • Be professional. You can deal with queries in a diplomatic and confidential manner.
  • Be willing to work flexible hours to meet service needs.
  • There is no personal care or administration of medication for this role.
Shift Pattern
  • 4‑day working week on a rolling rota (3 long days + 1 short day).
  • Short day: 9:00am – 5:00pm or 12:00pm – 8:00pm
  • 3 rest days after each working block.
  • Includes one weekend every three weeks.
  • Rotas provided 3 months in advance.
