Job summary

Stockport School is seeking an outstanding practitioner to join as Healthy and Thriving Intervention Coordinator - HLTA. The post supports coordinating and delivering targeted early intervention for students across our Healthy and Thriving Hub.

The successful candidate will work under the Healthy and Thriving Manager to identify needs, coordinate interventions, attend Inclusion panel meetings, and ensure timely, purposeful support to help students feel safe, understood, and ready to learn.

We are seeking to appoint an outstandingpractitioner to the post of Healthy and Thriving InterventionCoordinator - HLTA.

The successful candidate will bea dedicated and committed professional who, under the line management ofthe Healthy and Thriving Manager, will be responsible for supporting theeffective coordination and delivery of targeted early intervention for studentsacross our Healthy and Thriving Hub.

The Healthy and ThrivingIntervention Coordinator will play an important role in supporting theidentification of students’ presenting needs, coordinating appropriateinterventions and ensuring that students receive timely and purposeful supportto help them feel safe, understood, emotionally regulated, ready to learn andthrive.

The successful candidate will beexpected to attend and minute our Inclusion panel meetings, coordinateagreed actions and allocate students to appropriate interventions, ensuringthat baseline information, intervention goals and suitability of support areestablished. They will also monitor the progress of interventions and ensurethat appropriate review and exit processes are completed, working closelywith colleagues delivering interventions across our In-house Healthy andThriving Team and external support.

The Healthy and ThrivingIntervention Coordinator will also be expected to deliver targeted,personalised and goals-based early intervention to students who require focusedsupport, working individually or in small groups to develop emotionalwellbeing, regulation, resilience and positive approaches to learning.

In addition to the principalfocus of coordinating and quality assuring the school’s targeted interventionpathway, the Healthy and Thriving Intervention Coordinator will contribute tothe wider work of the Healthy and Thriving Team, working collaborativelywith Pastoral, SEND, Safeguarding, Attendance and Curriculum colleagues toensure that students receive appropriate and effective support.
