Job description
Role Summary

Deliver the Flying Start enhanced health visiting programme to a defined caseload of children, families and communities to support children to reach their full potential.

Main Duties
  • Assess health needs and develop, implement and evaluate programmes of care following the Flying Start Healthy Child Wales Programme.
  • Work with families and young children in their homes and community venues to provide advice, support and interventions to help parents make decisions positively affecting family future health and wellbeing.
  • Actively work in a multi‑agency environment and promote effective working relationships and communications with colleagues, professionals and statutory agencies.
  • Provide seamless service for children, families and communities and promote professional relationships with wider Flying Start Team, statutory and voluntary practitioners.
Qualifications
  • Health Visiting qualification and registration on the appropriate part of the NMC register.
  • Excellent communication skills and ability to work autonomously and as part of a team.
  • Desirable: ability to speak Welsh.
