Salary: £25,714.29 per annum

About The Role

Are you passionate about helping people overcome substance misuse and supporting mental wellbeing?

The Forward Trust delivers trauma-informed, gender-responsive Health and Wellbeing services across Surrey prisons in partnership with NHS healthcare providers, and we're looking for a dedicated Health & Wellbeing Practitioner to join our team at HMP Downview, a closed adult female prison.

What you'll do:
  • Manage a caseload, providing tailored assessments, care plans, and recovery‑oriented interventions.
  • Deliver structured 1:1 sessions and group-work, supporting clients with substance misuse and low‑level mental health needs.
  • Provide harm reduction guidance and second signatory duties.
  • Support release planning and transitional care for service users returning to the community.
  • Work within a multidisciplinary team, collaborating with mental health, healthcare, and prison staff.
  • Contribute to KPIs, audits, and the continuous improvement of our Health & Wellbeing services.
What we're looking for:
  • Experience in substance misuse, mental health, or related health and social care fields.
  • Strong interpersonal and communication skills, with the ability to work collaboratively.
  • Flexibility to work on a rota including weekends.
  • Ability to manage your caseload effectively and maintain accurate records.
  • Commitment to trauma‑informed, gender‑responsive, and recovery‑oriented approaches.
