The Health and Wellbeing Coach isa non-clinical / medical role and a new service introduced into the PCN throughexternal funding - Sport England Place Partnerships investment. The rolesupports patients to take control of their wellbeing and live happier, healthierlives through a focus on group clinics.You will be required to provide support to patients in a face to faceenvironment, as well as by email and telephone.
Deliver Group Clinics: Plan andfacilitate group sessions in community settings to support patients inimproving wellbeing and sustaining healthier lifestyles.
Empower Patients: Promote independence,self-management, and sustainable behaviour change through personalised supportand motivational techniques.
Facilitate Access to Resources: Connectindividuals with peer support, self-management tools, and community activitiesto build confidence, resilience, and social connection.
Collaborative Working: Partner withSocial Prescribers, Care Coordinators, Pharmacists, and widerclinical/community teams, including the Active Partnership, to link patients tolocal services that promote movement and wellbeing.
Person-Centred Support: Use coaching andmotivational interviewing to help individuals set and achieve meaningful healthgoals tailored to their needs and preferences.
Data-Informed Practice: Use frameworkssuch as Core20Plus5 to target interventions and support access to personalhealth budgets where appropriate.
Monitor and Evaluate: Conduct regularreviews, maintain accurate records, and contribute to service evaluation usingboth qualitative and quantitative methods. Report regularly to the PCN SteeringGroup with progress updates and measures.
Community Engagement: Activelyparticipate in local initiatives to promote health awareness and physicalactivity.
Professional Standards: Upholdconfidentiality, ethics, and a holistic, person-centred approach in allinteractions.

As a health and wellbeing coach you will be responsible for the delivery of an exciting new patient facing project running group health and wellbeing sessions in the community. You will have influence to shape the session content and target patient population based on data and research, help to identify appropriate venues in the community, and publicise sessions to our patients.
This role will suit individuals who enjoy taking the initiative, shaping new ideas, and delivering projects from start to finish. Aligned to national changes to Neighbourhood health, this is an exciting opportunity to lead a new preventative approach across Newport and Central.
This role involves a combination of daytime and evening hours, and we're looking for someone with the flexibility to support individuals at times that work best for them. As an employee of Newport and Central PCN your usual place of work will be our offices in Shawbirch, but work will be completed throughout the wider PCN footprint with a particular focus on neighbourhoods where there is greater need.
