Job summary

Cera is seeking an Extra Care Scheme Team Leader to oversee care staff and ensure the welfare and independence of service users in our extra care schemes. You will work under the Branch Manager to promote wellbeing, monitor care delivery, and update care plans to reflect changing needs.

You will coordinate with health services, support emergency care delivery, and encourage participation in activities, while adhering to safety and equal opportunities policies.

Qualifications

  • Knowledge of care planning procedures and procedures used in social care.
  • Experience updating care plans and maintaining accurate records.
  • Experience supervising care staff and promoting team accountability.
  • Ability to provide direct care in emergencies when required.
  • Experience coordinating with health services and external professionals.

Responsibilities

  • Produce and review care plans for all service users in accordance with procedures.
  • Monitor the delivery of care to ensure it meets required standards and inspections.
  • Monitor care staff and ensure care plans are regularly updated.
  • Provide direct care and support in emergencies to ensure appropriate packages of care.
  • Assist service users to access specialist care when needed, including nursing or healthcare.
  • Encourage and assist service users to participate in daytime activities and social engagements.
  • Liaise with health services and other professionals regarding patient care and medication administration.
  • Ensure manual handling and risk assessments comply with policies and procedures.

The role:

Reporting to the Branch Manager, the Extra Care Scheme Team Leader is responsible for the Care staff. Under the guidance of the Manager, the Extra Care Scheme Team Leader is responsible for ensuring the welfare of all service users. You will promote the independence, security and wellbeing of service users.

What we're looking for
  • Produce and review care plans for all service users in accordance with the associations care and planning procedures.
  • Monitor the delivery of care to ensure that it is delivered to an acceptable standard which satisfies the requirements of the Social Care inspection.
  • Monitor the work of care and to ensure that care plans are regularly updated and accurate records are maintained.
  • To provide, in an emergency, direct care and support to service users where appropriate, to ensure that they receive the right package of care and support which meets their identified needs.
  • Assisting the service user to access specialist care where appropriate, e: nursing or healthcare.
  • Encourage and assist service users to daytime activities, including day care, leisure, cultural, spiritual, recreational and social activities that will enhance their quality of life.
  • Liaise closely and effectively with all relevant professionals, care workers, families and advocates involved in providing service user care.
  • Liaise with health services in administering, recording and supervising medication in accordance with our policies and procedures.
  • Ensure that all manual handling and risk assessments are completed in accordance with Cera Care policies and procedures for all service users.
  • Carry out all duties in compliance with Equal Opportunities and Health and Safety policies.
  • Attend one to one supervision meetings, annual appraisals, staff meetings and training when required.
  • Work effectively as a Team Leader to promote and encourage independence amongst the tenants at all times.
  • Support with delivery of the on call rota, dependent on needs of the business.
  • This will include being the designated contact for the OOH teams during evening / weekends and supporting all aspects of care delivery.
Background and Experience
  • Knowledge of rota planning ensuring adequate staff on duty with correct skill mix and fair allocation of off duty/annual leave
  • Experience conducting Client Introductory visits
  • Ideally hold the RQF Diploma level 3 Health and Social Care
  • Experience of working with Service Users who have additional support needs
