Job summary

Cera is seeking an Extra Care Scheme Team Leader to coordinate care staff across schemes, ensuring welfare, independence and wellbeing for service users.

Reporting to the Branch Manager, you will develop and review care plans, monitor service delivery, supervise staff, liaise with health professionals and support the on‑call rota to meet client needs.

The role includes access to NVQ training, employee benefits, pension, and 25 days holiday, with a strong emphasis on person-centred care.

Qualifications

• Produce and review care plans for service users in line with procedures.
• Monitor care to ensure it meets required standards.
• Update care plans and maintain accurate records.
• Provide direct care in emergencies to meet identified needs.
• Assist service users to access specialist care when required.
• Encourage participation in activities to improve quality of life.
• Liaise with professionals, families and advocates involved in care.
• Administer, record and supervise medication per policies.

Responsibilities

• Update care plans and maintain records.
• Provide direct care in emergencies when needed.
• Assist access to specialist care where appropriate.
• Promote daytime activities to enhance quality of life.

• Ensure assessments and safety checks are completed.
• Attend supervision, appraisals and meetings as required.
• Work as a Team Leader to promote independence for tenants.
• Support on call rota according to business needs.
