Job summary

Nottinghamshire Hospice is seeking a Head of Community Services to lead the operational and clinical delivery of our community-based palliative care. You will ensure high-quality, safe, compassionate care and support people to live as well as possible at home.

As a member of the Hospice Leadership Team, you will provide visible leadership, drive performance, workforce planning and service development. Regular travel across Nottinghamshire and involvement in the on-call rota are required.

Responsibilities

• Lead operational and clinical delivery of community-based palliative care.
• Provide visible leadership within Hospice Leadership Team, drive care excellence and workforce development.
• Lead and develop Hospice in Your Home (24-hour service) and Wellbeing Day Services; ensure sustainability and patient-centred care.
• Travel across Nottinghamshire and participate in the clinical on-call rota (approx. one week every five weeks).
• Build partnerships with healthcare and community stakeholders to improve access and patient pathways.
• Oversee performance, resource management and workforce planning to meet patient needs.
• Champion equity, inclusion, diversity and belonging; foster a high-performing culture.
