Job description
Overview

A rare opportunity has arisen for a District Nurse or Community Nursing Sister to lead the Stour District Nursing Team, within the Christchurch Locality. The team has a breadth of experience and a shared goal to offer the highest standard of care to our patients. We work closely with other health and social care professionals across primary and secondary care, as well as with patients, families and carers in their own home.

Responsibilities
  • Lead the team and caseload manage the provision of care within an individual\'s home or residential home, including palliative care and long term conditions management.
  • Work autonomously using your own initiative to deliver holistic care to adults with physical health needs.
  • Provide supervision, mentorship, training and clinical support to students and trained practitioners.
  • Plan, implement and review health improvement programmes in a range of settings.
  • Provide specialist care and interventions, assessments, treatments and managing patients on a caseload, as part of the wider multi-disciplinary care team.
  • Provide specialist advice to others regarding the management and care of patients / service users.
  • Devise specialist programmes of care for other professionals to deliver and ensure they are delivered appropriately.
  • Ensure care delivery respects people\'s safety, privacy and dignity as a core component of a high quality, person-centred approach.
Qualifications and requirements
  • Ideally hold Specialist Practitioner District Nurse qualification or be willing to undertake in the future. Secondment opportunities could support future applications for this course.
  • Experience in caseload management and the ability to prioritise work.
  • Excellent leadership abilities, interpersonal skills and time management skills.
  • The role is full time and will include working weekends and bank holidays on a rota basis.
  • Applicants requiring Skilled worker sponsorship to work in the UK are ineligible.
  • Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of requiring a new check.
