Overview

An exciting opportunity has arisen for discharge keyworker to join UHD Discharge Team.

The post holder will be situated in the hospital discharge team, designated to clinical areas to support discharge planning according to hospital flow pressures. The post holder may be required to participate in cross-site working between Poole and Bournemouth hospital sites. The position will be required to work across a 7-day rota / the service operates on weekend and Bank holidays.

It is essential the successful candidate is able to be adaptable to different working priorities as the clinical area designated will change according to the hospital pressures been experienced on a daily basis.

The post holder will have well developed interpersonal skill as the role requires the individual to communicate effectively at all levels, demonstrating negotiating, problem solving and solution creating skills.
