Job summary

West London NHS Trust is seeking an experienced Discharge Co-ordinator to join the Hospital Discharge Team. You will manage a caseload and work with multidisciplinary teams, social care, and community services to remove barriers to discharge and reduce length of stay.

The role involves leading on complex discharge planning and promoting best practices to ensure safe, timely patient flow from admission to discharge.

Qualifications

• Experience in discharge planning and coordination across multidisciplinary teams.
• Strong clinical knowledge and understanding of hospital-to-community pathways.
• Excellent communication, leadership and problem-solving skills.

Responsibilities

• Facilitate timely, safe discharge of patients in liaison with ward staff, community services and social care.
• Attend daily meetings and participate in MDTs, professional and family discussions.
• Ensure Early Discharge Notification and Needs Based Assessments are completed promptly.
