Job summary

Royal Bournemouth and Christchurch Hospitals NHS Foundation Trust is seeking a Surgical Discharge Keyworker to coordinate and facilitate safe, timely discharge planning for surgical patients on the assigned ward. You will work with the multidisciplinary team, patients, families, carers and external providers to ensure smooth transitions from hospital to home or onward care.

Qualifications

• Experience coordinating patient discharge plans and pathways.
• Ability to identify barriers to discharge and develop mitigation plans.
• Strong communication with patients, families, carers and the care team.

Responsibilities

• Act as primary contact for discharge arrangements on the ward.
• Participate in daily discharge planning meetings and MDT rounds.
• Ensure a discharge plan is started on admission with a target discharge date.
• Coordinate with social services, community care and providers for timely discharge.
• Support cross-site collaboration across UCS Dorset NHS Foundation Trust sites.
