Job summary

Salford Council is seeking a dedicated worker to support discharges from the Bevan Unit within the Intermediate Care Social Work team. You will work under CAOs and Social Workers to locate suitable care homes for clients requiring 24-hour care and assist with tasks to facilitate timely discharges.

The role involves liaising with social care staff, nursing staff, and providers, maintaining clear records, and communicating complex information effectively, both verbally and in writing.

Qualifications

• Experience of maintaining accurate records and communicating complex information clearly, both verbally and in writing.
• Experience of working with adults/older people.
• Ideally experience in health and social care and working with other professionals.

Responsibilities

• Support discharges from the Bevan Unit under guidance from CAOs and Social Workers.
• Assist with care home finding for 24-hour care needs and tasks that facilitate prompt discharge.
