Job summary

Qualifications

• Strong communication and interpersonal skills with a proactive approach.

Responsibilities

• Promote and manage the Discharge to Assess Pathway with patients and families.
• Collaborate with ward teams to assess needs and coordinate services.
• Hold a caseload of complex patients and monitor progress.
• Participate in education for peers and other staff within the Trust.
• Provide supervision and support to allocated junior staff.
• Rotate as SMH Discharge Hub screener and contribute to governance meetings.
