Digital Clinical Hub Care Coordinator

The closing date is 22 December 2025

This role is designed to support GP practices within the Southport and Formby Primary Care Network (PCN) by helping with important administrative tasks. Working centrally at our main Southport office suite, as part of the Digital Clinical Hub team, you will assist in handling clinical correspondence for Southport and Formby GP practices.

We are looking for people who are digitally proficient and have some experience of working within healthcare. If you're eager to make a difference in primary care and work within a busy but supportive team, we would love to hear from you!

This role is part of a growing team within the PCN, and although advertised as a full-time role working Monday to Friday, part-time applications will be considered.

Main duties of the job

Key duties include but are not limited to:

  • Viewing documents received by the GP practices, and coding them using established templates and protocols.
  • Referring any documents that need further action to the appropriate GP practice team, following clear guidelines.

The role also involves training through the Personalised Care Institute, which will help you develop your skills as part of the Care Co‑Ordinators national framework.

You will be working in a growing team consisting of both clinical and non‑clinical colleagues, so teamwork and good communication skills are vital.

Southport and Formby PCN is a group of 14 GP Practices located in the Southport and Formby area who work together with other local organisations to improve the health and wellbeing for our local population of around 129,000 people.

You will play a key role in supporting the Digital Clinical Hub, ensuring that medical records are coded accurately and efficiently, and that actions are correctly identified and followed up on

Job responsibilities

Duties to include the following;

  • Follow a set protocol to decide which documents to process
  • Code and file documents received by GP practices
  • Code documents using an EMIS template created by the digital clinical hub to ensure the EMIS health record is clear and easy to understand.
  • Refer documents that require any action to the relevant GP practice team using the EMIS appointment booking page
  • Complete a daily audit by running a search in EMIS web to support data quality.
  • Complete a daily log of work done in Microsoft SharePoint
  • You will work to a daily target for the number of documents completed daily, which we will support you to achieve.
  • Engaging in weekly huddles to collaborate with team members, share knowledge and skills, and find ways to improve the service.
  • Attending relevant training to support both the development of the Digital Clinical Hub and your own personal growth.
