Job summary

Wye Valley NHS Trust seeks 2 full-time Band 3 Community Therapy Support Workers to join the Discharge to Assess service in Herefordshire. You will work with Occupational Therapists and Physiotherapists to deliver rehabilitation and reablement programmes in clients’ homes and care settings.

Visits include homes, Care Homes and bedded units across the South West, East and West of the county, with emphasis on independence, safety and patient empowerment.

Qualifications

• Functional Skills (Maths/English GCSE) at level 2 or equivalent
• 3 A-Levels or Health and Science Level 3 qualification
• Or Level 3 apprenticeship pathway in healthcare
• Equivalent clinical experience in AHP profession

Responsibilities

• Deliver direct therapy support under supervision by registered therapists
• Carry out delegated duties and contribute to service improvement
• Work with adults and/or children in community settings
• Carry own caseload after initial assessment
