Job summary

Wye Valley NHS Trust in Ross-on-Wye is seeking a Band 4 Therapy Assistant Practitioner to join the Therapy team at Ross Community Hospital. You will support physiotherapists, occupational therapists, and other therapy staff across medical-surgical wards.

The role involves independent and team assessments, developing exercise programmes, planning discharge, and helping prevent de-conditioning, with opportunities for service development and student education.

Qualifications

• Foundation degree in Health and Social Care or equivalent level 5.
• Care Certificate.
• Ability to work autonomously with complex cases and MDT input.

Responsibilities

• Assess patients independently and with the therapy team.
• Provide physiotherapy/occupational therapy assessments and interventions.
• Develop exercise programmes and patient rehabilitation.
• Plan discharge with personalised goals and involve patients in decision-making.
