Job summary

Wye Valley NHS Trust seeks 2 full-time Band 3 Community Therapy Support Workers to join the Discharge to Assess service in Herefordshire. You will work with Occupational Therapists and Physiotherapists to deliver rehabilitation and reablement programmes in clients’ homes and care settings.

Visits include homes, Care Homes and bedded units across the South West, East and West of the county, with emphasis on independence, safety and patient empowerment.

Qualifications

  • Functional Skills (Maths/English GCSE) at level 2 or equivalent
  • 3 A-Levels or Health and Science Level 3 qualification
  • Or Level 3 apprenticeship pathway in healthcare
  • Equivalent clinical experience in AHP profession

Responsibilities

Job summary

We are seeking 2 full time, motivated & caring Band 3 Community Therapy Support Worker to join our Discharge to Assess (D2A) Service.

This is an exciting opportunity to work within a multidisciplinary community team supporting people to return home safely following discharge from hospital.

You will work under the supervision of Occupational Therapists and Physiotherapists to deliver rehabilitation & reablement programmes that help individuals regain independence, improve confidence, achieving their personal goals within their own home environment.

The role is ideal for someone who is passionate about supporting people to remain independent & who enjoys working in a fast-paced, rewarding community setting.

Visiting clients in their own homes as well as Care Homes and bedded units in and around the South West, East and West of the County based on service demands.

The post holder will need to be

The post holder will support a range of interventions, including home equipment provision and fitting.

Main duties of the job

Working as part of the therapy and wider multidisciplinary team, therapy support workers use developed skills under the supervision of registered therapists and may also work alone and with other non-registered therapy staff. Tasks and responsibilities include the direct delivery of clinical, technical or scientific activities following the completion of role specific competencies. Therapy support workers are able to make fact-based judgements and suggest treatment progressions to a registered therapist.

This role requires knowledge and understanding of facts, principles, processes and general concepts in their field of work. The post holder will carry out a wider range of delegated duties and will have responsibilities, with supervision and guidance available when needed. Post holder will contribute to service improvement and are responsible for their own development. Therapy support workers will carry their own caseloads that have already been seen by a registered professional, prioritise their own workload, and work with service users under a range of supervisory arrangements while undertaking routine processes.

The post holder will be required to complete the relevant competencies required for the post. This post may include working with adults and/or children.

Car driver essential.

Proficient knowledge and use of IT systems required.

Education & Qualifications
Essential
  • Functional Skills (level 2) or Maths & English GCSE (grades 9-4 or A*-C)
  • 3 A-Levels or Health and Science T-Level or equivalent level 3 qualification(s)
  • OR as a level 3 apprenticeship (e.g. Senior Healthcare Support Worker apprenticeship (or other level 3 qualification pathway)
  • OR equivalent clinical experience/expertise within AHP profession underpinned by theoretical and practical experience
Desirable
  • Care Certificate

Essential
  • Knowledge of specific relevant patient conditions and therapy interventions acquired through experience
  • Experience of multi-disciplinary record keeping
  • Basic Computer Skills including word processing and e-mail
  • Able to make clinical judgements without direct supervision
  • Able to manage busy case load and prioritise
  • Ability to work independently (under guidance) using own knowledge skills and initiative acknowledging role boundaries
