Job summary

Wye Valley NHS Trust in Herefordshire is seeking two full-time Band 3 Community Therapy Support Workers to join the Discharge to Assess service. You will work with Occupational Therapists and Physiotherapists to support patients to regain independence at home after hospital discharge.

You will visit clients in their own homes and care homes across the county, provide rehabilitation and reablement support, and help with equipment fitting.

Qualifications

• Functional Skills level 2 or maths/english GCSE.
• Three A-Levels or Health and Science T-Level or equivalent.
• Or level 3 apprenticeship pathway.
• Equivalent clinical experience underpinned by theory.
• Care Certificate.

Responsibilities

• Deliver direct clinical, technical or scientific activities under supervision.
• Make fact-based judgements and suggest treatment progressions.
• Carry own caseload under guidance.
• Complete relevant competencies for the post.
• Support service improvement and staff development.
• Work with adults and/or children.
