We are a Band 5 Staff Nurse. Our service operates from 8 am to 8 pm, seven days a week, offering flexibility in working hours. An additional team of nurses and HCAs offers a robust, reactive evening and night nursing team.

Main duties of the job

You will be responsible for providing skilled nursing care and working flexibly to support the team in delivering high‑quality integrated care.

You will need to demonstrate excellent communication and interpersonal skills, with the ability to be adaptable, reliable, resourceful and approachable.

Providing integrated care following a holistic assessment which enables patients to be cared for in their own homes and clinic settings. You will facilitate the prevention of unnecessary hospital admission/re‑admission by reducing the effects of ill health and disability and maximising the individual's potential for independence.

A robust induction and competency‑based development package will be provided.

Job responsibilities
  • Assess, plan, implement, promote and evaluate safe, creative and effective treatment plans in collaboration with the patient’s diverse needs, carers and other professionals.
  • Deliver evidence‑based interventions.
  • Be responsible for the safe monitoring, management and administration of medication.
  • Work autonomously, building leadership skills and advanced clinical assessment skills.
  • Build effective relationships with patients, their carers, family and colleagues across the MDT.
  • Develop exceptional planning and organisational skills.
  • Analyse care and treatment provided, and develop sound clinical decision making.
  • Be trained to deliver high quality care and develop knowledge in all areas including palliative care, leg ulcer management, acute and chronic wound care, bladder and bowel dysfunctions, and provide supportive care to carers and family members.
  • Delegate and support junior staff, building leadership skills.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
