The successful applicant will be a registered nurse with community nursing experience and a high standard of caseload management. You will possess exceptional people skills and be able to work as part of a multi‑disciplinary team.

Support and Development

You will be provided with support tailored to your needs to drive forward high quality care and teamwork. This includes a thorough induction programme, regular one‑to‑ones and peer supervision.

Responsibilities

• Provide evidence‑based care to a defined population within a Primary Care Network.
• Manage caseload, delegate effectively and monitor care standards.
• Lead and line‑manage less experienced nurses and other team members to deliver high quality care.
• Contribute to continuous improvement of community nursing, linked to the 10‑year NHS plan.
