Job summary

The South Nottinghamshire Adult Community Hub within Nottinghamshire Healthcare NHS Foundation Trust is seeking a proactive Service Advisor (Band 4) to manage referrals, handle high volumes of calls, and signpost service users to the right care.

This 20-hour per week, fixed-term role (6 months) based at Lings Bar Hospital, Gamston, Nottingham, requires accuracy, IT skills, and the ability to work with multidisciplinary teams.

Qualifications

• Educated to GCSE standard or equivalent
• Able to demonstrate literacy and numeracy
• NVQ Level 4 in Business Administration or equivalent experience

Responsibilities

• Manage and process incoming referrals to the Hub, address queries, take detailed messages, and refer them to the relevant multi‑disciplinary team member as defined in the referral management protocols.
• Ensure service users/patients are directed to appropriate community assessment support services.
• Effectively use communication skills to enable referrers to explain needs and required assistance, while being sensitive to their feelings and respecting their wishes whenever possible.
• Assess referrals to determine the type and priority of need, and appropriately refer to other services and agencies, seeking advice from the line manager if necessary.
• Ensure all referral documentation contains relevant information to assist clinical triage.
• Input data into electronic clinical records to support the flow of information between the Hub, clinical teams, and partner agencies.
• Keep electronic systems up to date by entering and amending patient admissions, discharges, and other details to allow audit, review information, and produce relevant reports.
• Contribute to the provision of administration services to adult care teams, prioritising and managing the workload efficiently.
• Manage communications with other healthcare professionals in accordance with local and Trust Policy.
• Support clinicians by collating patient information in preparation for Multidisciplinary Team meetings.
• Ensure confidentiality in all matters related to patients and information obtained during employment.
