Job summary

Nottinghamshire County Council seeks a committed individual to provide prompt information, advice, and access to adult social care and public health services. You will represent the department to the public and external agencies with a focus on high-quality customer care, information gathering, and efficient referrals.

Responsibilities include triaging inquiries, signposting to services, and maintaining accurate records while supporting colleagues and partner organisations to meet service

Qualifications

• GCSE English and Maths at A-C or equivalent.
• Knowledge of current Adult Social Care and Health policy drivers (e.g., Think Local, Act Personal).
• Awareness of the functions of Adult Social Care and Health services and ability to access information.

Responsibilities

• Provide high quality, courteous advice and referral service to the public and partners.
• Signpost and advise on services from local authority and voluntary/statutory agencies.
• Offer specialist social care guidance to frontline workers and health staff.
• Triage referrals and determine outcomes, seeking guidance when needed.
• Maintain accurate records and ensure data quality.
• Attend team meetings and contribute to service improvement.
