Nottinghamshire Healthcare NHS Foundation Trust is seeking an administrator to manage and process incoming referrals to the Hub, address queries, take detailed messages, and direct them to the appropriate multidisciplinary team member as defined in referral protocols.

You will input data into electronic clinical records to support the flow of information between the Hub, clinical teams and partner agencies, keep systems up to date, and assist with MDT preparations while maintaining patient

Job description

Manage and process incoming referrals to the Hub, address queries, take detailed messages, and respond or refer them to the relevant multi-disciplinary team member as defined in the referral management protocols. Ensure service users/patients are directed to appropriate community assessment support services. Effectively use communication skills to enable referrers to explain needs and required assistance, while being sensitive to their feelings and respecting their wishes whenever possible. Assess referrals to determine the type and priority of need, and appropriately refer to other services and agencies, seeking advice from the line manager if necessary. Ensure all referral documentation contains relevant information to assist clinical triage. Input data into electronic clinical records to support the flow of information between the Hub, clinical teams, and partner agencies. Keep electronic systems up to date by entering and amending patient admissions, discharges, and other details to allow audit, review information, and produce relevant reports. Contribute to the provision of administration services to adult care teams, prioritising and managing the workload efficiently. Manage communications with other healthcare professionals in accordance with local and Trust Policy. Support clinicians by collating patient information in preparation for Multidisciplinary Team meetings. Ensure confidentiality in all matters related to patients and information obtained during employment. Please note that this post does not meet the pay level required for a Skilled worker visa. Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa. Applicants requiring a Skilled Worker Visa can determine the likelihood of obtaining a Certificate of Sponsorship against the relevant criteria here https://www.gov.uk/skilled-worker-visa Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment. You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.
