Job Summary

Are you an established Occupational Therapist, Mental Health Nurse or Practitioner looking to work in the community? Intensive Community Support Dementia team would like to hear from you! The community role covers North and West Dorset, being a driver is essential for this role, pool cars are available. As one of the Clinical Team Leaders you will support patients in a specialist service where Community Mental Health Teams can refer when a patient’s behaviour becomes more challenging. We offer high quality interventions to help patients in their own environment, which may be a care/residential home or in their own private home. You will work with CMHTs, social care and other agencies to benefit patients and support carers while patients are under our care and signpost for ongoing support. You will be highly motivated and confident in clinical decision-making, managing shifts and using resources effectively.

Responsibilities
  • Understanding of MHA, MCA and safeguarding legislation.
  • Undertake assessments and nurse reviews, manage a small group of patients, triage referrals and coordinate shifts with other Clinical Team Leaders.
  • Implement service changes as guided by the National Dementia Strategy and embrace the Recovery Agenda as it is developed and applied to organic services.
  • Process information quickly as part of a virtual ward and coordinate with senior nursing staff.
  • Complete a Prevention and Management of Violence and Aggression course; failure to complete means the post cannot be commenced.
Essential Qualifications
  • Registered and valid Mental Practitioner Nurse, Occupational Therapist or Social Worker.
  • Evidence of continuous professional development.
  • Able to assess, plan, implement and evaluate programmes of care; risk assessment and management.
  • Ability to prioritise and meet deadlines; organise work of self and others; supervise, teach and assess; self‑motivated and able to motivate others.
  • Uses initiative and can make decisions; manages difficult situations effectively.
  • Works well in a multi‑disciplinary team.
  • Demonstrable skills in written and spoken English adequate for the role.
Desirable Qualifications
  • Evidence of working with patients with a dementia diagnosis and older persons.
  • Evidence of working in the community.
  • Competent computer skills, experience inputting information on RIO.
  • Experience of duty work/shift coordination.
  • Experience of lone working.
Travel
  • Subject to the provisions of the Disability Discrimination Act, able to travel using own vehicle on Trust business.
