A healthcare provider in Tasmania is seeking a Clinical Nurse Consultant to support older Australians in the transition from hospital to residential care. You will collaborate with interdisciplinary teams, provide expert advice, and help facilitate a smooth discharge process. This full-time position includes a salary range of $114,851 to $121,886 with numerous benefits including salary packaging and professional development opportunities.
The residential aged care hospital liaison clinical nurse consultants (RACHL’s)assist in co-ordination between hospitals and residential facilities, facilitating a smooth discharge process from hospital to a residential aged care home. Working in partnership with the dynamic hospital aged care liaison (HALT) & transition to Care program (TCP) multidisciplinaryleadershipteams.

Fixed term, full time, day worker position, working up to 76hours per fortnight, 7 February 2026 until 31 October 2028.

$114,851 to $121,886 per annum. Our Employer 12% superannuation contribution is on top of this amount.
In addition, the following allowances calculated on the salaried incremental point may be available:
You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.
