Job summary

A healthcare provider in Tasmania is seeking a Clinical Nurse Consultant to support older Australians in the transition from hospital to residential care. You will collaborate with interdisciplinary teams, provide expert advice, and help facilitate a smooth discharge process. This full-time position includes a salary range of $114,851 to $121,886 with numerous benefits including salary packaging and professional development opportunities.

Job description
The Team

The residential aged care hospital liaison clinical nurse consultants (RACHL’s)assist in co-ordination between hospitals and residential facilities, facilitating a smooth discharge process from hospital to a residential aged care home. Working in partnership with the dynamic hospital aged care liaison (HALT) & transition to Care program (TCP) multidisciplinaryleadershipteams.

The Role
  • Supporting Older Australians
  • Support patients and their families with the residential care process.
  • Aiming to support staff within the Launceston General Hospital with the process referrals to my aged care (MAC) portal, prospective resident profile documentation, Dementia Services Australia and transfer to residential care.
  • Provide expert advice and leadership within an interdisciplinary team undertaking comprehensive patient assessment, care coordination and implementation of planned transfer of care to and from RACHs from within the inpatient hospital setting to enable care needs to be met in the most appropriate environment.
  • Undertake a broad consultative and educational role by initiating and maintaining a comprehensive care network, building, and maintaining links with external providers including RACH staff in addition to medical, nursing, allied health, and administrative staff.
  • Act autonomously and as part of a team to be a conduit between the hospital and RACHs, including at times of significant operational demands

Details of appointment

Fixed term, full time, day worker position, working up to 76hours per fortnight, 7 February 2026 until 31 October 2028.

*Notwithstanding hours to be negotiated with the successful applicant.

Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time, part time, and casual vacancies.

Salary

$114,851 to $121,886 per annum. Our Employer 12% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmania State Service) Agreement 2023.

In addition, the following allowances calculated on the salaried incremental point may be available:

  • Post Graduate Allowance of up to 10 % based on qualification type
Salary Packaging

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.

Eligibility

Successful applicants will be required to meet the essential criteria

  • Registered with the Nursing and Midwifery Board of Australia as a Registered Nurse

Applicants should note the following criteria are desirable:

  • Holds or is working towards a relevant post graduate tertiary qualification and/or recent extensive nursing experience relevant to the role.
