Job summary

A healthcare organization in St Albans is looking for a dedicated professional to manage client care and support. Job description
The Role
  • Management of clients and responsibility for their ongoing care and support with a focus on person centred care.
  • Undertake competency assessments and/or onsite support of Care Professionals in the field as needed to support clients.
  • Meet and carry out assessments with new clients, and complete all paperwork as required.
  • Achieve agreed KPI’s for Care Professional and client touchpoints.
  • Create, update and audit client care plans.
  • Maintain accurate client records on Home Instead’s systems.
  • Conduct client and Care Professional introductions.
  • Build and maintain positive relationships with potential and existing clients, their families and other professionals involved in their care.
  • Carry out client Service Reviews, Quality Assurance visits and address any resulting actions.
  • If required, support, mentor and supervise Care Professionals in relation to their client’s needs and outcomes.
  • Provide cover for absence of others in the care and care professional supervisory team.
  • Participate in on-call duties (weekday evenings and occasional weekends).
  • Ensure compliance with Home Instead’s Equality, Diversity and Equal Opportunities Policy, in respect of employment and service delivery.
  • Carry out any other duties deemed necessary for the successful operation of the business.
  • Delivery of care when required.
