Job summary

OneMedical Group is seeking a Care Navigator to help patients access the right care and services. You will receive, review and process patient requests, ensuring information is recorded accurately and directed to the appropriate clinical or administrative pathway.

You will help ensure patients experience an efficient, supportive and seamless journey when using our services, working with clinical and administrative colleagues to manage requests and improve processes.

Qualifications

• Excellent written and verbal communication
• High accuracy and attention to detail
• Solid IT skills and willingness to learn clinical/digital systems
• Organised, methodical, and able to work independently
• Ability to follow defined processes and escalation procedures
• Awareness of confidentiality, information governance and data protection
• Capacity to manage workload and meet timescales

Responsibilities

• Review and process incoming patient requests via digital and approved channels
• Assess and route requests using Anima or other approved systems
• Signpost patients to appropriate practice, community, external, or self-referral services
• Escalate clinical concerns to a clinician when required
• Record and maintain complete patient information in clinical systems
• Collaborate with clinical and administrative teams to manage requests
• Support service improvements in access, signposting, workflows and data quality
• Maintain strict confidentiality and comply with governance policies
